SWIM LESSON NOTICES TO DOWNLOAD AND PRINT

HOW TO SUBMIT AN EMAIL REGISTRATION
- Download a registration form for the correct Learn to Swim Program.
- Save the downloaded Swim Lesson Registration Form in Microsoft Word format as: email registration form child’s first name.doc
- Complete the form by using Microsoft Word and then save it again after all the information is filled out; PLEASE INDICATE A FIRST AND SECOND CHOICE when requested.
- Return the completed form as an attachment and use your last name as the subject. Send it to: jhesketh@theworkoutclub.com
- DO NOT SEND CREDIT CARD INFORMATION ON THIS FORM (not a secure site).
- You will receive a telephone or email confirmation within 2-3 business days at which time you can make payment over the phone or arrange for payment at the club in person. If you do not receive a confirmation within this time frame, CALL THE AQUATIC CENTER IMMEDIATELY AT 434-6565 extension 28.
- Download the Parent Information form for the correct Learn to Swim Program to record your lesson day and time after receiving confirmation.
